Current Openings:    SSVF Health Navigator

The Support Services for Veteran Families (SSVF) Health Navigator works directly with homeless Veterans and Veterans at risk for homelessness, and their families, who are eligible for services through COVO SSVF. This position works collaboratively with the COVO SSVF team and other service provider multi-disciplinary teams that assist participants in navigating through complex resources and processes to obtain/sustain housing.  The SSVF health care navigator provides medical and mental health care coordination, health education, multi-disciplinary collaboration and consultation, and administrative duties required by the program. The SSVF Healthcare Navigator works closely with the Veteran’s care provider and members of the Veteran’s assigned interdisciplinary treatment team.
